School-Based Health Centers

The Waterbury Health Department and Waterbury Public Schools are bringing medical, dental, and behavioral health services directly to city students—right at school. School-Based Health Centers are clinics located inside the school to allow students to see a nurse practitioner, dentist, or counselor during the school day.

Waterbury Public Schools with School-Based Health Centers

Once a student is signed up for the Waterbury School-Based Health Centers, they can be seen at school with or without their parent or guardian present. Parents and guardians consent to the services they want their child to receive and they don’t have to give up their own doctor to go to the school-based clinic. All students can be seen regardless of insurance status. Any student in Waterbury can be seen at any of the locations below—they don’t have to go to that school. 

Services are provided in partnership with Community Health Center, Inc. The Community Health Center staff works with school nurses and the student’s own doctor to provide medical, behavioral health, and dental services for students during the school day. Sign up today!

School-Based Health Service Insurance

The clinic takes most insurance plans for medical, dental, and behavioral health care. Insurance is billed for services. For uninsured students, the school-based health center team will connect families to the Access to Care team. The Access to Care team will help with the application for public health insurance programs or other insurance coverage that the student may qualify for. Uninsured students who are not eligible for a public health insurance program can receive care at the school-based health centers at a reduced rate through our sliding scale fees. 

All children enrolled in a school-based health program are eligible to receive care services, regardless of insurance status or ability to pay.

Yearly Physicals and Immunizations for Students

Following Connecticut General statutes, each student enrolled in the Waterbury school system is required to have a physical according to the following schedule:

  • Health assessment before initial entrance into preschool programs
  • Health assessment before initial entrance into kindergarten
  • Health assessment in grades 6 and 10
  • For transfer students, health assessment before initial entrance into the Waterbury schools
  • For ungraded students, health assessment before initial entrance and whenever a health assessment is required for students of the same age cohort as the ungraded student
